If you would like to attend Sunday morning worship services at Maricopa Alliance Church but you do not have transportation, we will be offering shuttle services soon.
In order to request shuttle service, please read the Waiver of Liability and Hold Harmless Transportation Agreement below and complete the Registration form. By completing the Registration form, you agree to the terms of the waiver.
We will contact you via phone Sunday mornings to verify your plans to attend each week.
Waiver of Liability and Hold Harmless Transportation Agreement
1. I hereby release, waive, discharge and covenant not to sue the Maricopa Alliance Church, the Board and its individual members, officers, agents, servants, or employees (hereinafter referred to as releasees) from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my minor child(ren) or me, or any of the property belonging to me, as result of, or in any way arising out of myself or my child(ren) traveling to and or from Maricopa Alliance Church and/or Saddleback Elementary School sponsored activities in a vehicle or vehicles not owned or operated by the District.
2. I voluntarily assume full responsibility for any risks of loss.
3. I further hereby agree to indemnify and hold harmless the releasees from any loss, liability, damage or costs due to myself or my child(ren) traveling to and or from Maricopa Alliance Church and/or Saddleback Elementary School sponsored activities in a vehicle or vehicles not owned or operated by the District.
4. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed and enforced in accordance with the laws of the state of Arizona.
5. In completing the Registration form, I acknowledge and represent that I have read the foregoing Waiver of Liability and Hold Harmless Agreement, understand it and sign it voluntarily as my own free act and deed.
